Barry Keoghan has confirmed he underwent a noninvasive skin-tightening procedure, a step that has generated a sharp divide among his followers. The actor, known for roles in Eternals and Saltburn, appeared in a video posted last week by Dermaspa founder Nilam Holmes. In the clip, Keoghan is seen receiving a XERF treatment on his face. He stated in a voiceover that he feels better about himself and believes it is important to look after oneself both inside and out.

Holmes, who runs the clinic, identified the session as Keoghan’s second XERF treatment. She noted that she has worked with the actor for some time across various film and campaign projects. According to the clinic’s website, XERF is designed to be comfortable and does not require needles or numbing cream. The procedure targets mild to moderate skin laxity. While some clients see results after one session, the clinic encourages two treatments to extend the initial effect.

The cost of the procedure is substantial. Dermaspa lists a single face and neck session at approximately $3,300. Two treatments cost around $6,000. Keoghan acknowledged the therapeutic nature of his visits, saying the experience is relaxing every time he comes in. Holmes added that the actor’s skin has improved over the last few years and that he offered to let her film the process.

The reaction from fans was starkly split. Some users expressed surprise, arguing that Keoghan looked handsome in his films and did not need to do anything. One commenter suggested that Hollywood pressures actors to look perfect, but if the treatment boosts confidence, there is no harm in it. Others praised the actor for being open about the procedure, describing his skin as flawless. A different fan joked about getting the treatment when they win the lottery.

Keoghan has previously spoken about the intensity of online criticism regarding his appearance. In a March SiriusXM interview, he described the abuse as making him shy away from attending places or going outside. He said the hate is past the point where everyone simply goes through it. About a month later, he confirmed on a podcast that he had stopped socializing and attending events. This withdrawal followed unconfirmed rumors about his relationship with pop star Sabrina Carpenter prior to their 2024 split.

The actor said the narrative about the breakup was never confirmed by him, leading him to disappear from public view. He is now 34 and currently working on projects that may raise his profile. He is set to play the Joker opposite Robert Pattinson in The Batman: Part II, which is due for release in 2028. He has also been spotted in costume as Ringo Starr for Sam Mendes’ upcoming four-part Beatles films.

Those films will also star Harrison Dickinson as John Lennon, Paul Mescal as Paul McCartney, and Joseph Quinn as George Harrison. Keoghan’s recent disclosure adds a new layer to his public image as he prepares for these major roles. The XERF procedure remains a popular option for those seeking noninvasive improvements to skin texture and tightness.
